The Truth is in the Stars is a Secondary Quest in The Witcher 3 Wild Hunt. Secondary Quests are optional Quests not required to finish the game. Some of these quests may have a bearing on certain Main Quests or may be a consequence of another Quest.
The Truth is in the Stars can be obtained by collecting the notice off of the Notice Board in Oxenfurt or speaking to the old sage marked with a yellow exclamation mark in Benek. The old sage asks Geralt to fetch some food for him in exchange for reading his future.
Deep in the swampy woods of Velen there is a roadside altar praising the glories of one of the forgotten gods. That is where Geralt encountered a shriveled, half-senile old man who claimed he could foretell the future and would read Geralt's fortune - for a small fee.
