Bombs in The Witcher 3: Wild Hunt refer to thrown projectiles Geralt can use to supplement his other combat techniques, abilities and tools. When thrown, Bombs produce a variety of effects including dealing damage in an area, hindering the abilities of Monsters or inflicting crowd control effects on targets. Rounding out Geralt's diverse set of monster hunting tools, Bombs, just like Potions, Decoctions and Oils can be crafted through Alchemy and their usage and effects are heavily tied to the Alchemy Skill line.

Bomb Crafting
Bomb creation is is a skill mastered by Witchers through years and years of honing the craft and the study of Alchemy, as well as subjecting themselves to various toxins and chemical compounds to better understand their properties. Bombs can be crafted using various Alchemy Ingredients found throughout the environment, obtained from opponents or purchased from Alchemists and Herbalists in cities, villages and other settlements.

In order to craft a Bomb, you must first learn its Formula by finding and reading its Manuscript Page — these are crafting instructions similar to Weapon and Armor Diagrams and each one is specific to every single Bomb. Many manuscript pages can be found randomly in chests and guarded treasure throughout the world, while some can be purchased from Alchemists and Herbalists. Few even have fixed locations or specific points in the game where they can be obtained.
All Bombs also have upgraded versions with the Enhanced and Superior prefix. Upgraded versions of Bombs typically have improved effects, extended durations for those with effects that persist, and number of charges. Each upgraded version also has its own Manuscript Page that you will need to find and the forumlas for these will require that you have at least 1 unit of the previous tier Bomb in your inventory. For example, while you may be able to find the Manuscript for Superior Devil's Puffball early on, you will not be able to upgrade to it directly from the basic Devil's Puffball as the Enhanced Devil's Puffball is required as one of the crafting components.
Bomb Usage & Monster Nests
Once crafted, Bombs can be equipped to your Bombs Quickslots for easy usage in combat. Up to 2 types of Bombs can be equipped this way but you can swap your equipped Bombs at any time.
Bombs can be readied and aimed manually for precision throws to hit specific targets or groups of targets. Investing in the Steady Aim skill in the Alchemy Skill line allows you to slow down time while aiming Bombs, in order to better hit your targets. By default, only some Bombs have a damaging component with the rest producing varied effects that make combating Monsters easier. However, players can also invest in the Pyrotechnics skill to not only boost the damage dealt by damage-dealing Bombs, but also add a damaging component to those that normally do not do damage.
Some Bombs have the special function of being able to destroy Monster Nests. These are places Geralt may come across where a certain type of Monster will keep spawning indefinitely until the nest is destroyed. The Bombs that qualify for this function are as follows:

Bomb Replenishment
Just like Potions and Decoctions, each Bomb has a set number of Charges. Enhanced and Superior versions of Bombs typically have more charges along with other improved benefits. You can also invest in the Efficiency skill in the Alchemy Skill line. At max level, this skill provides up to 5 additional charges for all bombs. Once all charges are used up, you will not be able to use the Bomb again. However, you do not need to craft Bombs over and over. Instead, charges can be replenished by Meditating for at least One Hour while you have some Strong Alcohol in your inventory. Strong Alcohol refers to high concentration alcoholic spirits that can be used for alchemical purposes. The following items qualify for this purpose:
Note that you only need 1 unit of any of the above to refill all of your Bomb charges via Meditation. This will also refill all of your Potions and Decoctions. These alcoholic beverages can commonly be found throughout the world and they can also be purchased from Merchants. Some can even be crafted through Alchemy using other components. Dwarven Spirit in particular is quite abundant so you should rarely be found wanting for these.

The Herbalist ( outside Oxenfurt, northeast hut) -- Mentioned for Devil's Puffball also has Dragon's Dream, Golden Oriole, and Enhanced White Honey (none of which I've ever found in loot.) If someone better with these chart edits could add these I'd appreciate it.
