- Possible Location: Velen - no mans land at a Fisherman's house to the left of The Spoils of War sunken chest. A Bridge with some soldiers is to the right of Spoils of War. Go north of the fisherman house to his boat jetty. Dive in the water and it's in a sunken chest.
- Possible Location: Velen - Bandit camp on the top of a hill Northeast of Lindenvale, up inside the loot chest.
- Confirmed location: Towerful of Mice Quest. Go to the upper most level where the prisoners are held. Exit and go to the far side of the ledge to find a chest with this diagram. Accessible from level 6.
