Disturbance is a Main Quest in The Witcher 3 Wild Hunt. Main Quests are the main storyline of the game that can have a major effect towards the endings. Some of these quests may have dialogue choices that will alter the ending of the game.
First, ascend to the guest room. You will witness and hear a magical explosion just as you touch the ground, followed by Yennefer cursing. When you get close, a cutscene will appear where she will explain that "this was the third crystal" and that she needs to make a quick call.
Vesemir told Geralt that Yennefer had set up her trusty megascope as soon as she arrived at Kaer Morhen, but could not get it to function for the life of her. This news greatly troubled the witcher. If Yennefer was planning to use her megascope, that meant a conversation with other sorceresses was in the works – and that in turn meant scheming, politics and other things Geralt tryly could not stand.
As expected, Yennefer did not extend Geralt the warmest of greetings. Nevertheless, Geralt, ever the noble and patient soul, helped the sorceress eliminate the source of the magical disturbance. It turned out the culprit was a case of dimeritium bombs Lambert had carelessly placed near the sorceress' room. Tsk, tsk, Lambert.

Walkthrough for The Witcher 3 Disturbance
Enter the fortress at Kaer Morhen and head up the spiral steps of the tower. You will come across a not too happy Yennefer. Speak with her and you have 2 choices. You can either ask her what she needs you to do which continues the quest or tell her you heard what she did with the bed at which point she becomes angered and teleports you over the lake. You'll have to swim back and enter the fortress to continue the quest.
Your mission will be to help Eskel and Lambert as well as fix the megascope. Yennefer will give you a potestaquisitor to find the energy disturbance. Equip it as a selected item and start hunting. Head to the grand hall towards Vesemir and Uma. The energy is coming from boxes by a steel cage left of Vesemir. Inspect them and ask Vesemir what's in them. He offers to remove the bombs.
Return to Yen and explain the disturbance. She summons Ida to discuss Uma's curse and she tells you about Ciri's blood being vital. Answer however you wish. After a history about the migration the quest is finished and you may resume the other 2 quests with Eskel and Lambert or continue to No Place Like Home.
