The Truth is in the Stars is a Secondary Quest in The Witcher 3 Wild Hunt. Secondary Quests are optional Quests not required to finish the game. Some of these quests may have a bearing on certain Main Quests or may be a consequence of another Quest.
The Truth is in the Stars can be obtained by collecting the notice from the Notice Board in Oxenfurt or speaking to the old sage marked with a yellow exclamation mark in Benek. The old sage asks Geralt to fetch some food for him in exchange for reading his future.
Deep in the swampy woods of Velen there is a roadside altar praising the glories of one of the forgotten gods. That is where Geralt encountered a shriveled, half-senile old man who claimed he could foretell the future and would read Geralt's fortune - for a small fee.

Walkthrough for The Witcher 3 The Truth is in the Stars
Once you started this quest, look for an old man on the west side of the village, you'll see him outside his hut and he'll comment that he has been expecting you. Interact with him and he'll reveal that he can see visions but his seeing eye is not as sharp as it used to be due to old age. He will then ask you to find dragonsroot for him in exchange for a vision from him.
You can find the dragonsroot southeast from the village inside a cave on the coast. Be wary of the Ghouls guarding the cave, there are four of them. Kill the ghouls guarding the cave then activate your witcher senses to find the dragonsroot in the area. Once you've acquired the dragonsroot, go back to the old man and give it to him. He will then say some things related to Ithlinne's Prophecy, then the quest will be concluded.
